SecurBox® DECLARATION FORM

  1. Notice: The owner of any goods ship by way of SecurBox must make a full and accurate declaration, either personally or by his/her consigner, on this form of the whole of the unaccompanied contents or goods contain within the SecurBox. All such goods contained within the SecurBox must be described in the appropriate category below and must be accompanied by genuine invoices of purchase.
  2. Prohibited and restricted goods. The importation of certain goods is prohibited or restricted by the Laws of The Bahamas. Failure to declare such goods may involve transfer to manifest without notice. Such goods include fruit or other plant materials, animals, birds, dogs, firearms and indecent or obscene articles. Commercial goods are also prohibited for shipment by SecurBox on the Parcel List. Goods valued at $2,500.00 and over must be manifested-not allowed on Parcel list.  
  3. Insurance. Items registered on the Parcel List are shipped at owner's risk. Please note: SecurBox(s) are to be closed and sealed in the owner's presence.

DECLARATION

We, the Consignor and Consignee, hereby, jointly and severally, declare that the goods described below (and overleaf) are a true and accurate description of the contents of the SecurBox shipped aboard the M/V Betty K for importation at ARAWAK CAY PORT, NASSAU ,THE BAHAMAS.

We further declare that the goods so shipped are intended solely for the Consignee's personal or household use, and that all such goods contained in the SecurBox have been packed therein by the Consignor, as agent of the Consignee, with  the Consignee's express knowledge and consent.

By signing this form, we the Consignor and Consignee, jointly and severally agree to hold Betty K Agencies (USA) LLC and Betty K Agencies Limited, harmless for all and any damage, suit or action arising out of a false declaration made by us and to fully indemnify them in respect of any damage or cost occasioned or incurred by them as a result of a false declaration.
